The Hospice Team
Medical Director - Certifies the clinical prognosis and terminal illness diagnosis. Leads the hospice team in developing a plan of care. Provides consultation to the care team.
Skilled Registered Nurses - Evaluates the patient's condition during regular scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques, the use of medication, and pain management.
Social Worker - Assesses social needs of the patient and family and provides psychosocial services and counseling.
Chaplain - Provides spiritual support. Works as a liaison with clergy of the patient's choice. Coordinates with team for family bereavement support.
Certified Nursing Assistant - Provides non-medical care under the supervision of the RN by attending to patients' personal care. They help patients with bathing, grooming, and other activities of daily living.
Volunteer Services - Volunteer Coordinator works with a team of trained volunteers to help provide comfort and companionship, when needed.
